Operational dashboard issue tracking and integrity review
Purpose
Dashboard integrity reports track whether operational dashboards have known issues, stale data, broken assumptions, or resolved findings. This keeps reporting problems visible instead of hiding them in individual user notes.

Admin Practice
When a dashboard number looks wrong, record the issue, source report, suspected data source, owner, and fix path. Close the issue only after the dashboard and underlying report agree with the corrected data.
